Skip the clinic and stay home! We bring the exam room to you. Our Concierge style Home Physicals provides a full, comprehensive evaluation in the comfort of your own space. Whether your child needs a physical for school, sports, or camp, Dr. Sarah Slater, DNP, delivers expert, compassionate care right to your doorstep. Each visit includes: - A head-to-toe physical exam - Medical history review - Completion of school, sports, or camp forms - Discussion of any findings or concerns - Convenient, stress-free care at home Consultation fees: - $50 concierge/travel fee (applies once per visit) Physical Exam fee: - First child: $100 - Second child: $75 - Each additional child: $50 *If a health concern is found and your child is not cleared for participation, the child's physical exam fee will be waived. You will only be charged the concierge fee. Appropriate guidance or referrals will be provided.
